Enter your home address or a place that you can safely receive mail. This may be a Post Office address (P.O. Box) if that is how you receive your mail. If your mail will be sent to someone other than yourself, please include an “In Care of Name” as a part of your mailing address. If your mailing address is a U.S. territory and it contains an urbanization name, list the urbanization name in the "In Care of Name" box.
Your physical address is the location where you live. Please provide a complete address. Every U.S. address includes a street address with a number and street name. You may or may not have an apartment number. All addresses should include the city, state and ZIP code.

Indicate your employment status below. Employment is not a requirement to act as an I-134 sponsor. However, you will need to demonstrate you have the resources to support the beneficiary.
To support the beneficiary, it is necessary to demonstrate you have sufficient income or financial resources. Our software will evaluate you based on your income only. You are not required to list assets, but doing so will help strengthen your declaration.
Create a record for each dependent in the supporter's household. The household generally includes the spouse, dependent children, and any other individuals the supporter financially supports. Do not list the beneficiary or any persons in the beneficiary's household.
In addition to providing financial support, it is recommendd that you make specific contributions to cover the beneficiary's basic living needs while in the U.S. For example, you may furnish room and board or monetary support.
